Position Summary:
We are seeking a detail-oriented and empathetic Intake Coordinator to manage the initial onboarding process for new patients. This role is critical in ensuring a smooth transition into care, coordinating with physicians, referral sources, and internal departments to facilitate timely and compliant admissions.

Key Responsibilities:

• Receive and process referrals from hospitals, clinics, and physicians
• Verify insurance eligibility and obtain necessary authorizations
• Coordinate with clinical staff to schedule initial assessments
• Communicate with patients and families to gather intake information and explain services
• Ensure all documentation complies with state, federal, and agency regulations
• Maintain accurate records in EMR (DSL) systems and track referral outcomes
• Collaborate with interdisciplinary teams to support seamless patient onboarding

Qualifications:

• High school diploma or equivalent required; associate or bachelor’s degree preferred
• Minimum 2-3 years of experience in home health, hospice, or healthcare intake
• Familiarity with Medicare, Medicaid, and private insurance guidelines
• Strong organizational and communication skills
• Proficiency in EMR specifically DSL system and Microsoft Office Suite
• Ability to multitask in a fast-paced environment with empathy and professionalism
